Activity Sheets:
Complete an activity sheet for each school and community activity, service group or volunteer activity
listed on your resume. Use a blue form for any school related activity, service and leadership.
Use the yellow form for any outside community service and leadership.
Each sheet must be signed by the corresponding adviser, coach, sponsor or mentor, verifying your active
participation.
Any volunteer service or school activity listed on your resume that does not have supporting
documentation will not be considered.
Confidential References (5):
Five confidential letters of recommendation referencing your character, leadership and service
are to be submitted. At least two (2) of the letters must be from classroom teachers and two from
individuals who know you outside the classroom ( coaches, advisors, ministers, community service /
volunteer coordinators, employers.) . The last is student's choice.
Letters from immediate family members will not be accepted.
It is suggested that letters of recommendation come from individuals with whom you have been
associated in the past two years and who know you well.
Please provide a Confidential Reference form and your resume to each letter writer.
Although a student may have qualified scholastically for consideration for membership in the Signum Laudis Chapter of the National Honor Society, this does not guarantee selection to membership. It is the candidate's responsibility to see that all of the supporting documentation is submitted to the Main Office / Attn: Honor Society advisor by the stated deadline. Late or incomplete candidate folders will not be considered.
After review of the completed folders submitted on time, the Faculty Council will vote to determine membership. The Faculty Council vote is final and may not be appealed except in the case of procedural error.
