Going forward, please fill out this Primary School Pick-Up form for your Primary School child if they will be picked up from school. Please do not send in notes for future dates. This form is only for the Primary School and will NOT work for other school buildings.
Forms MUST be submitted by 12:00 PM on the day of pick-up. After this time, forms will no longer be accepted and your child will be placed on the bus. If your child is not designated as a Permanent Pick-Up with the Main Office you will need to fill out this form each time before they are picked up.
